How to Register:
Download and complete the Dance Across Birmingham Registration Form. Only complete one form per person. The Form can be completed electronically and submitted by mail or fax. Please note that two signatures are required. A Parent or Guardian must complete the form for anyone under the age of 18 regardless of how it is submitted.
To Submit by Mail or Fax:
Once you have completed your form electronically, simply click the "Print Form" button at the bottom of the page. Your registration information will be saved and printed. Mail your form to: Alabama Dance Council, P.O. Box 2126, Birmingham, AL 35201. Or fax to (205) 979-5214. Remember to check the boxes in the Release Statements sections to accept the terms and conditions. Signatures are required for mailed or faxed forms.
